Carrollton Youth Football, Inc. recognizes the challenges many families
face when balancing their financial obligations. To assist you in meeting
your financial obligations, we are pleased to offer a couple of financial
options. For the payment plan option and/or a Scholarship consideration
please e-mail your request to Michele York. Please note
Payment Plan/Scholarship Inquiry in the Subject line. Our scholarship
committee will review the applications and will notify all applicants of
the decisions by July 15, 2008.

Payment Plan
In order for a payment plan to be considered the following documentation has to be turned in:

1. Registration Form, Waiver and Code of Conduct; (download)
2. Birth Certificate;
   
3. The uniform cost in the amount of $60.00
4. All open balances must be paid in full by July 26th

Scholarship Program
In order for your scholarship application to be considered the following documentation has to be turned in:

1. Registration Form, Waiver and Code of Conduct; (download)
   
2. Birth Certificate;
   
3. Scholarship application; (Download it here!)
   
4. Free/reduced Lunch letter from the previous year; and
   
5. The uniform cost in the amount of $60.00.

Applicants will be notifed the week of July 14th.
